Individual registration must be submitted on the appropriate
form. Print the form, fill in the appropriate information and mail
in to Smudger’s Soccer Camps with the fee for that particular camp.

For team registrations please reserve places by phone and send in team application forms together.

Please make checks* payable
to Smudger’s Soccer Camps
and send along with a self addressed stamped envelope for
a written confirmation.

*Lakewood Camp (Camp #4, June 16 - 20 and Camp # 10, August 4 - 8) are held at Ridgewood Belcher Recreation Center. A mandatory membership charge of $2.00 is included in the registration fee. Please make the checks ($122)
